Free rate confirmation template with a filled example

In short

A rate confirmation needs seven things on one page: the parties and references, each stop with its date and appointment window, the freight, the rate with fuel stated in or out, the accessorial terms, the instructions the driver works from, and the legal terms carried in from the carrier agreement. This template has all seven, with the four fields that start disputes made explicit.

Instructions

Check in with the pickup number. Get the BOL stamped in and out at both stops. Status texts go to the driver mobile above: tap the link to share location. Photograph the signed POD at delivery and send it before leaving. Seal the trailer at pickup and record the seal number on the BOL. Call (972) 555-0100 for any delay over 30 minutes.

Load status communications

Carrier agrees, on its own behalf and on behalf of its drivers and dispatchers assigned to loads tendered by Marrowdale Logistics, that Marrowdale Logistics and its service providers may send automated text messages and place automated or artificial-voice calls to the mobile numbers Carrier provides, for the purpose of confirming assignment, requesting the load's location and status, and confirming delivery. Message frequency depends on the load and will not exceed four texts and two calls per load per day. Message and data rates may apply. Any recipient may stop text messages at any time by replying STOP, may stop automated calls by saying so on a call or by contacting Marrowdale Logistics, and may reply HELP for assistance. Consent is not a condition of doing business with Marrowdale Logistics.

Terms

This confirmation is subject to the Broker-Carrier Agreement between the parties. Carrier shall not re-broker, co-broker, or assign this load to another carrier. Payment terms: net 30 (quick pay 2%, 3 days) from receipt of a legible, signed proof of delivery and the carrier's invoice. Accessorial charges are payable only when stated above or approved in writing before they are incurred, and when supported by documentation: facility in/out times for detention and layover, the receipt for a lumper, the dispatch record for a truck ordered not used. Carrier confirms the equipment is clean and in working order and that the driver has the hours of service to meet the appointments above. Any change to the appointments must be confirmed with the broker before the driver departs.

Template by LogiPilot · logipilot.ai/templates/rate-confirmation · A template, not legal advice; have counsel review your terms.

The example above is filled in the way a careful brokerage fills it: appointment windows with times, fuel stated as included, detention free time and rate spelled out, a truck-ordered-not-used amount, and who pays the lumper. The blank version keeps every field and the clause language; put your own terms in, and have counsel look at the terms section once.

The "Load status communications" paragraph is the consent clause that lets a brokerage text and call drivers automatically for status. It is the same wording published on the carrier consent page and used by LogiPilot Ping customers. Delete it if you do not send automated messages.

FAQ

Questions about the rate confirmation template.

Can I use this rate confirmation template for my brokerage?

Yes. It is free to use and modify. Replace the broker details, keep or adjust the accessorial terms, and have your counsel review the terms section and the consent clause once before you send it to carriers.

Does a rate confirmation need to be signed?

Most broker-carrier agreements make a rate confirmation binding when the carrier signs it or accepts it electronically, and some when the carrier dispatches on it without objection. Get a signature or an emailed acceptance anyway; it removes the argument.

Why is the consent clause on a rate confirmation?

Automated status texts and artificial-voice calls to drivers require the recipient's prior express consent under US law. The rate confirmation is the document the carrier signs on every load, so it is where brokerages put the language, in addition to the carrier agreement.
