Free carrier packet template: onboarding checklist and carrier profile

In short

A carrier packet collects what a brokerage needs before the first load: the signed broker-carrier agreement, a W-9, a certificate of insurance naming the broker, a copy of the operating authority, payment details or a factor's notice of assignment, and working contacts. This template is the checklist and the profile form on two pages, with the safety and identity checks that stop double brokering written into it.

Load status communications

Carrier agrees, on its own behalf and on behalf of its drivers and dispatchers assigned to loads tendered by Marrowdale Logistics, that Marrowdale Logistics and its service providers may send automated text messages and place automated or artificial-voice calls to the mobile numbers Carrier provides, for the purpose of confirming assignment, requesting the load's location and status, and confirming delivery. Message frequency depends on the load and will not exceed four texts and two calls per load per day. Message and data rates may apply. Any recipient may stop text messages at any time by replying STOP, may stop automated calls by saying so on a call or by contacting Marrowdale Logistics, and may reply HELP for assistance. Consent is not a condition of doing business with Marrowdale Logistics.

Certification

The undersigned certifies that the carrier holds active operating authority and the insurance stated above, that it will not re-broker loads tendered by Marrowdale Logistics, and that the information in this packet is accurate.

Onboarding services collect most of this automatically; the packet still matters because it is where the carrier signs the things that govern the relationship, including the no-re-brokering promise and, if you track loads by text, the consent clause. The identity line in the checklist, that contact numbers match FMCSA records or are confirmed by calling the FMCSA-listed number, is the single cheapest fraud check a brokerage can run.

FAQ

Questions about the carrier setup packet template.

What insurance should a carrier packet require?

Auto liability of at least $1,000,000 and cargo coverage of at least $100,000 are the common minimums for dry van freight, with the broker named as certificate holder so it is notified of cancellation. Reefer and high-value freight often require more cargo coverage; check the shipper's contract.

How old should a carrier's authority be?

Many brokers require ninety days or more of active authority before the first load, because brand-new authorities are the profile most often used in freight fraud. Exceptions are made for carriers with a known principal or a reference; the packet should say who approved the exception.
